What Is HbA1c and Why It Matters?

HbA1c is one of the main blood tests used to understand glucose levels over time. To understand what the number means, it helps to start with what is actually being measured.
Red blood cells circulate through the bloodstream and contain a protein called hemoglobin. Glucose, the sugar measured in blood glucose tests, can attach to that hemoglobin. This process is called glycation. Once glucose has attached to hemoglobin, that hemoglobin is described as glycated.
It should be kept in mind that blood glucose is not static. The body continually balances glucose production, glucose uptake by tissues, and insulin secretion. A glucose measurement taken at one point in time reflects one part of that process.
HbA1c adds a different perspective because it measures the percentage of hemoglobin in circulating red blood cells that has become glycated. In simple terms, it tells us how much of the hemoglobin being measured has had glucose attached to it.
This is why HbA1c is connected to blood sugar. When glucose exposure is higher over time, more hemoglobin can become glycated. The result therefore provides information about glucose exposure over a period of weeks rather than only at the moment the blood sample is taken. It is also one of the established measurements used to classify glycemic status, including normal glucose regulation, prediabetes, and diabetes.
That distinction is central to understanding why HbA1c is useful.
It is also important to acknowledge that high blood sugar and low blood sugar are different clinical situations and need to be considered from different perspectives. Low blood sugar is known as hypoglycemia. It is important in its own right, including when individual glucose targets are being considered.
However, this article focuses specifically on what HbA1c can tell us about longer-term glucose exposure and elevated blood sugar.

Why HbA1c Reflects the Previous 8 to 12 Weeks

Red blood cells do not remain in circulation indefinitely. Mature red blood cells have an average lifespan of about 120 days. During the time they circulate, glucose can continue to attach to the hemoglobin inside them.
HbA1c takes advantage of this biology. Because the test measures glycated hemoglobin across circulating red blood cells of different ages, the result reflects glucose exposure over approximately the previous 8 to 12 weeks.
It is common to describe HbA1c as a “three-month average,” but that shorthand can be misleading if it suggests that every week contributes equally to the final result.
HbA1c is better understood as a weighted average. More recent glucose exposure contributes more heavily than glucose exposure further back in time. According to the research, glucose levels during the most recent 30 days account for approximately 50% of the final HbA1c value.
So, an HbA1c result is not a simple arithmetic average of every glucose level over three months. It is a longer-term measure in which recent weeks have greater influence.
The way the test works also creates an important limitation. Because HbA1c depends on red blood cells and their lifespan, factors that change how long those cells remain in circulation can also affect the result.
That is where interpreting HbA1c becomes more nuanced.

What a Higher HbA1c Can Tell You

Now that we have a better grasp of the context, the next question is what a higher result can actually tell us.
As glucose exposure increases over time, more glucose can attach to hemoglobin. This means that a higher HbA1c generally reflects higher average blood glucose over the period represented by the test.
That relationship makes HbA1c useful for identifying patterns of persistently elevated blood sugar. Standard diagnostic criteria classify HbA1c results into ranges associated with normal glucose regulation, prediabetes, and diabetes. An HbA1c below 5.7%falls below the prediabetes threshold, 5.7% to 6.4%is classified within the prediabetes range, and 6.5% or higherfalls within the diabetes diagnostic range.
These numbers need to be understood for what they are: diagnostic criteria, not universal personal targets. Targets used for someone who has already been diagnosed with diabetes are a different concept and can be individualized according to clinical circumstances.
A result also needs clinical context before it becomes a diagnosis. In the absence of clear symptoms of high blood sugar, the research specifies that a result in the diabetes range should be confirmed with repeat testing.
In addition, what HbA1c does not reveal on its own is what caused that higher result. The result is a measurement of a pattern, not a complete explanation of its cause.
Even so, there is another useful way to express that same pattern.

HbA1c and Estimated Average Glucose

HbA1c is reported as a percentage, but blood glucose measurements are usually expressed as a concentration, such as milligrams per deciliter (mg/dL) or millimoles per liter (mmol/L).
To connect these two ways of looking at glucose, HbA1c can be translated into something called estimated average glucose, or eAG. eAG is a calculated estimate of the average blood glucose level associated with a particular HbA1c value. Research has established a mathematical relationship between HbA1c and average glucose, allowing an HbA1c percentage to be expressed in the same units commonly used for blood glucose measurements.
The word estimated matters.
eAG does not mean that every rise and fall in blood glucose over the previous several weeks has been measured and then averaged together. It is calculated from the HbA1c result using the established relationship between HbA1c and average glucose.
This also helps clarify what people mean when they describe HbA1c as an “average blood sugar” test. eAG translates that result into an estimated glucose concentration.
But there is an important reason the relationship between HbA1c and average glucose is not identical in every situation. HbA1c depends not only on glucose exposure, but also on the red blood cells carrying the hemoglobin being measured, which makes the lifespan of those cells matter.

When HbA1c Can Look Higher or Lower Than Expected

The relationship between red blood cell lifespan and glycation has a simple but important consequence: the amount of time a red blood cell remains in circulation can influence how much opportunity there is for its hemoglobin to become glycated.
If red blood cells remain in circulation longer than usual, they have more time to accumulate glycation. HbA1c can therefore appear higher relative to the person’s actual average glucose.
The research identifies iron deficiency anemiaand vitamin B12 deficiencyamong conditions that can prolong red blood cell survival and raise HbA1c relative to average glucose. The same effect can occur after splenectomy, the surgical removal of the spleen.
The opposite can also happen.
When red blood cells are cleared or replaced more quickly, there is less time for glycation to accumulate. HbA1c can then appear lower relative to average glucose.
The research identifies several situations in which this can occur, including hemolytic anemia, where red blood cells are destroyed more rapidly; pregnancy; acute blood loss; blood transfusion; and erythropoietin therapy, a treatment that affects red blood cell production.
This does not make HbA1c an unreliable test. It shows why understanding what the test measures matters.
As the measurement depends on red blood cells, changes in the lifespan or turnover of those cells can also change the relationship between HbA1c and average glucose.
This is one reason an HbA1c result may sometimes need to be considered alongside other information rather than interpreted as an isolated number.

HbA1c Is One View of Blood Sugar

HbA1c gives a longer-term view of glucose exposure, but other tests look at blood sugar from different perspectives.
One of the most common is fasting plasma glucose, or FPG. Another is the oral glucose tolerance test, or OGTT.
These tests are related, but they do not measure the same thing in the same way:
  • HbA1creflects glucose exposure over several weeks.
  • Fasting plasma glucosemeasures glucose after an overnight fast.
  • OGTTmeasures the response to a standardized glucose load.
The research therefore treats them as complementary measurements of glycemic status.
As we have seen, HbA1c can sometimes be influenced by factors affecting red blood cells. More broadly, the point is not that one test is better than the others. Each provides a different view of glucose regulation.
This additional context can matter when an HbA1c result is unexpected.

Looking at HbA1c Over Time

A single HbA1c result gives you one measurement in your health record. Previous results add history.
Keeping HbA1c measurements from different dates together makes it easier to see whether the value has remained similar, moved higher, or moved lower over time.
That history does not explain the cause of a change by itself. But it can provide useful context when a new result is reviewed, especially when repeat testing is needed to clarify a finding.
